Funeral Package - When having to plan a funeral at St. Patrick, please feel free to review this before meeting with the priest to plan the service.
Instructions for completing these online A.C.T.S. registration forms:
- After downloading, please completely fill in all the fields on this form on your PC.
- Select the “Submit by Email” button in the upper right of the form
- After email message with an attachment is generated, send the email. This will enable the A.C.T.S. Retreat Director to build an accurate retreatant database
- After email is sent, select the “Print Form” button in the upper right of the form and then print the form.
- You may then attach your Retreat fee payment to the form and mail them to the address indicated or drop the form and payment by the Church office.
- It is suggested that you “save” the completed registration form on your PC for future reference.
