Requirements.

An Applicant Must:

• Complete an ONLINE screening form.

• Read the Program Booklet that outlines all screening instructions and provides the supervision and behavior guidelines each person agrees to adhere to when serving in St. Patrick Church or St. Patrick School.

• Sign an "Acknowledgement Form" that the applicant has read, understood and will comply with all Program requirements.

• Name three persons (not members of your immediate family, and who have known you at least 3 years) to serve as references.  If you provide the names of your references AND their email address when you complete the ONLINE Screening Form, you do not need to download a reference form to give to them.  However, if you do NOT provided email addresses for one or more references, you must download a reference form, complete the top portion, give it to your reference, and have them return it to the church.

• Participate in a "face-to-face" interview.

• Attend a "Family of Faith" training session.

• Submit to a criminal background check (automatically updated every two years for persons who remain active) and done only after all other requirements have been completed.

• Not serve in any capacity until they have received a written email notification that they have been "cleared" to serve (or a phone call or other message, if the applicant does not have an email address.)
