Instructions for completing the online A.C.T.S. registration forms:

• These forms are in Adobe .PDF format and can be edited on your computer.
• Click on the appropriate form and download it to your PC.

• After downloading, please completely fill in all the fields.

• Once the form has been completed, you can either send a copy via email or print it out and mail it to the address listed on the form.  Select the appropriate button at the upper right of the form.
• If by Email: Select the “Submit by Email” button in the upper right of the form. If your computer has a default email application you should see composed email message with the form attached.

• Send the email. This will enable the A.C.T.S. Retreat Director to build an accurate retreatant database.

• If by Mail: Select the “Print Form” button in the upper right of the form and then print the form.

• You may attach your Retreat fee payment to the form and mail them to the address indicated on the form, or you may drop the form and payment by the Church office.

• It is suggested that you “save” the completed registration form on your PC for future reference.
